Concussion otherwise known as mild Traumatic brain injury (mTBI) is a serious health problem affecting our society today. In fact, according to a research carried out by the Department of Health New Jersey, it was estimated that about 12,000 residents of New Jersey suffer traumatic brain injuries yearly, of which an average of 1000 are termed severe.
What is a concussion?
A concussion or mTBI is a type of brain injury that commonly occurs when a persons suffers a direct force impact to the head. Concussion is the most common form of head injury for athletes. It is associated with disorientation, and sometimes with loss of consciousness (LOC) followed by amnesia (forgetting) of what happened both immediately before and after the injury. However, it is important to note that it is not necessary to lose consciousness to have a concussion. Mild confusion or disorientation about who or where you are, what the time or date is, what you were doing when the injury happened, or a persistent headache can be signs of concussion.
There are two common types of injury to the brain in sports: Acceleration-Deceleration and Rotational. Acceleration-Deceleration Injury usually happens when the athlete's head is traveling at a certain speed and then abruptly stops. When this happens, the brain can hit the inside of the skull and brush against bony structures damaging delicate brain tissue. Rotational Injury happens because the brain is attached at its base where it joins the spinal column. Hits to the head or body may cause rotational motion of the brain within the CSF. This type of injury often leads to shearing of the brain nerve cells. So, you don’t have to hit your head to sustain a concussion.
Concussions however were once thought to be minor, but retrospect has proven that patients who do not seek follow-up medical care may be susceptible to second-impact syndrome, leading to conditions such as disrupted brain function and other debilitating long-term effects. Second Impact Syndrome is a rare, yet serious and possibly fatal, disorder, which occurs when a young athlete has not yet recovered from a concussion and then within a short period of time (usually within one week) receives a second blow to the head. In such cases, it is possible for rapid brain deterioration and even death to occur as the brain is not yet fully recovered from the first injury and the second injury causes rapid swelling in the skull.

- The athlete must feel 100%, asymptomatic from the concussion, as he or she felt PRIOR to the concussion.
- Post-concussion neurocognitive testing should be at or better than baseline testing. If there is no baseline, test results should have plateaued and are no longer improving.
- The athlete must be back to all cognitive activity (at school or work without accommodations that may have been provided as a result of the concussion) with no exacerbation of symptoms.
- The athlete should pass physical exertional testing, ideally by a certified team of professional with concussion expertise, and have completed the Zurich protocol with no return of symptoms.
- For youth, we recommend waiting three weeks with no symptoms before returning to sport or gym, although the youth may do reconditioning, non-contact exercise under the direction of the strength and conditioning coach.
- For athletes with a history of other recent or multiple concussions, a longer period of avoidance of contact risk may be recommended. In some cases, consideration may be given as to the risk of re-concussion and whether it is advisable to return to the sport.
